Workflow Forms are structured, multi-step forms triggered automatically at key moments in the client journey — before a first appointment, at check-in, after treatment completion, or on a recurring schedule. Build forms using text inputs, multiple choice, yes/no toggles, rating scales, image upload, and consent checkboxes. Every completed form attaches automatically to the relevant appointment record, is flagged for staff review where required, and is permanently stored in the client's medical file.

Navigate to Settings then Workflow Forms and click Create New Form.

Add your fields by choosing from: text input, multiple choice, yes or no, rating scale, image upload, or consent checkbox.

Configure the trigger for this form: send before the appointment, display at check-in, or request post-treatment.

Specify which service types or appointment categories this form applies to.

Activate — clients receive the form automatically via their preferred channel, either email link or SMS link.
